IIRC it's more of a set screw than a through bolt. You just put a little dimple in the actual frame, then the set screw goes through the brackets and into the dimple in the frame providing enough friction that the bracket could never slide around again.
For heavier setups, to prevent the brackets from moving, I believe Andersen recommends drilling a screw into the frame. I haven't done that, over 2 years and thousands of KMs towing its stayed in the exact same spot.

I can't see any high idling, what are your actual RPMs? Are you sure it's idling high and not just the noise of the fan you're hearing? At 240f the fan should be on.
These trucks are programmed to run hot, you can see on the gauges that oil temp is still well within what Ram considers "normal"...
In addition to Mega HO's point about everybody having different numbers and you need to compare your own instead, also keep in mind that there is a large difference between 9 and 10 mpg. Only sounds like 1 insignificant mpg, but I prefer to think of it in percentages. There's like 10% difference...
Very hard to say, if that's 8400 pounds dry then you're going to exceed GCWR with the 3.21. But yes it definitely helps if you plan to tow flat and take it slow and easy as the ratings are done under some pretty tough conditions which you may never see (towing up the davis dam)...
