Torque converter went to heaven at 26,000 miles

I own a 2022 2500 Cummins with a 68RFE trans. Had a check engine light come on and noted the exhaust brake was not working, took it to the dealership under warranty. Hoping it was just a flash update....wrong - 3 weeks later picked it up. On the work order stated: Torque conv. failure, OVERDRIVE CLUTCH BURNED AND OVER CVI SPEC. They installed torque conv. core, clutch , some sealer, filters, input shaft seal, ATF4, and o ring pump cover. I do some light towing (horse trailer) and throw st;uff in the bed no heavier than 1000 pounds. With only 26K miles on it, what would have caused such a failure at only 26K miles? Maintenance does'nt recommend anything on the trans at 26K. Trans fluid is checked every couple months etc... Glad it was under warranty $$$$

Anyone else have a similar issue and how do I prevent another failure. Is this a one off? Ive owned other diesels trucks and have never had this kind of failure.

Its not a common thing likely an issue with manufacturing the torque converter and the clutch failed and you lost lockup
 
Sorry this happened, probably just an unlucky situation.
 
The saga continues, brought it home from the dealership, no check engine light and exhaust brake working in full, automatic and tested the haul/tow mode. Parked it and went out later, now check engine light and NO exhaust brake in any mode. Took video and pic of the drive if the dealership thinks Im nuts.
Took it back to dealership yesterday, and was called today by service guy, Now its the valve body.
Googled signs and symptoms of a valve body going bad, it didnt include what is wrong with my truck. Shifted up and down fine. Part is on nation wide order and delay of part til wednesday next week.

On a side note had the transmission fill tube recall done a while ago. When all the problems started, I attempted to check the trans fluid. The replacement trans dipstick is only about 18" long and states on the dipstick.... "not for checking oil level".
Ram takes the old dipstick and sends it back to wherever, Might as well use a rubber cork, for all the use the new dipstick does. So if anyone has the recall done keep ask to keep the old dipstick or pull it before you have the recall done. I found this on a thread somewhere.

There is a recall for the recall on the dipstick where they change the dipstick tube and give you a new full length locking dipstick
